  2. Email the completed form to tournaments@pmaha.com with the tournament name in the subject line
  3. In order for your application to be considered, payment via interac or cheque must be submitted (see forms for details)

The Port Moody Minor Hockey Association acknowledges that we play and enjoy hockey on Indigenous lands.
